Provider-based status is a Medicare designation which treats an offsite location of a main provider hospital as a hospital outpatient department.
The decision to fully integrate Citizens Medical Center and Citizens Cardiovascular Associates was made to help ensure continued high quality care throughout the community.
Provider-based clinics are common across hospital systems and follow federal billing rules.
Citizens Cardiovascular Associates will become an outpatient department of and fully integrated with Citizens Medical Center on Monday, June 29, 2026.
The integrated model gives patients access to hospital resources, specialty care, diagnostic services, and connected medical records.
Provider-based clinics may bill a hospital facility fee and a physician professional fee, depending on insurance coverage.
These locations are now integrated with Citizens Medical Center as Citizens Cardiovascular Associates.
The location operates as a provider-based clinic, which can include hospital outpatient department billing.
Cost depends on your insurance coverage. Patients should review their insurance plan to understand out-of-pocket expenses.
